This is a link for certified Assistive Technology Professionals (ATPs) to schedule a virtual wheelchair evaluation with a therapist. The linked form will collect some basic information and will have a place to upload documents or images. After submitting the form, you can choose a date and time for the virtual evaluation that suits you and the patient. 

Before scheduling an appointment, make sure you already have the following materials:

• A referral or an equipment order from the patient's medical provider.
• The patient's insurance information.
• A document with patient demographics; DOB, diagnosis, address, phone number, Etc.
• A device that supports live video calls, like a smart phone, tablet, or laptop with a camera.  
• It is critical to ensure the patient is discharged from home health services prior to the visit. 

Our therapists use the Therapy Achievements Wheelchair Evaluation Letterhead to document all evaluations with justifications and a thorough assessment. It is a tried and true format that has resulted in a high rate of acceptance among payors. 

FAQs:

What Insurances cover this evaluation? 

• We are in-network with most payors; excluding only Medicaid and United Healthcare. A full list of in-network payors is linked here.

Can the ATP or the Patient pay out of pocket for the evaluation? 

• Yes. The cost of the evaluation is $250 and will be collected in advance.

My patient's payor for the wheelchair is AL Medicaid. Will the therapist fill out the appropriate form?

• Yes. Our therapists are familiar with the AL Medicaid form. 

Will Home Health interfere with payment for this evaluation?

• Yes. If the patient is recieving home health services, including any nurses, therapists, bath aids, or wound care professionals that visit the home of the patient and charge insurance for their skilled services, then the evaluation will be paid for out of pocket; $250. Insurance will pay for this evaluation if the patient is discharged from all home health services.
