This is a link for certified Assistive Technology Professionals (ATPs) to schedule a virtual or in-person wheelchair evaluation with a therapist. The linked form will collect some basic information and will have a place to upload documents or images. After submitting the form, you can choose a date and time for the virtual evaluation that suits you and the patient.
Before scheduling an appointment, make sure you already have the following materials:
- A referral or an equipment order from the patient's medical provider.
- The patient's insurance information.
- A document with patient demographics; DOB, diagnosis, address, phone number, Etc.
- A device that supports live video calls, like a smart phone, tablet, or laptop with a camera.
- It is critical to ensure the patient is discharged from home health services prior to the visit.
